Enrollment Procedure

We are excited to welcome all new and returning students! Follow the steps below to begin your child's enrollment journey

Step 1: Application

Secure a printed application form from the Registrar's Office, and fill out the form.

Step 2: Submission of Requirements

Please prepare the following documents for submission:

• PSA / NSO Birth Certificate (1 photocopy & original to verify)

• Report Card (Form 138) from previous school

• Certificate of Good Moral Character (if transferee)

• 2x2 or 1x1 recent ID pictures (2-4 copies)

• Medical Certificate / Vaccination Record

• Parent / Guardian Valid ID (photocopy)

• Completed Enrollment Form (available at the Registrar's Office)

• Proof of Payment of Registration Fee
