To ensure a smooth enrollment process, please prepare the necessary documents listed below. These help us verify your child’s eligibility and maintain accurate school records for the new school year.

Tuition & Fees

  • PSA Birth Certificate

  • Certificate of Completion from Preschool

For Kindergarten Applicants:

  • PSA Birth Certificate

  • Good Moral Certificate

  • Form 137 / Report Card (SF9)

  • Photocopy of Kindergarten Certificate of Completion

For Grade School Applicants:

  • Medical Certificate (Eyes, Nose, Ears & Physical Exam)

  • 1x1 or 2x2 ID Picture

  • Down payment for enrollment
